A step-by-step guide to the financial decisions that matter most, with a track for each stage: new attendings and early-career providers, dentists and dental practice owners, and established providers planning for retirement. Each step tells you what to ask and what to watch for.
The Practice Wealth Strategies Financial Playbook
2026 edition · 3 tracks · 9 pages- Track A: New attendings. Contract questions, own-occupation disability, term life, professional mortgages, your 401(k), 403(b), 457(b) and Roth options, and student loans.

- Track C: Established providers. Tax strategies, alternatives, asset protection, estate planning, practice transition and retirement income.
Includes 2026 contribution limits and a list of documents to bring to your consultation.
Long-Term Disability Insurance
Your ability to practice is your largest financial asset. A healthcare provider who earns $350,000 a year for 30 years will earn over $10 million. We protect that with coverage written for your profession and specialty.
True own-occupation
Benefits are paid if you cannot perform the duties of your own occupation or specialty, even if you can work in another role.
- Specialty-specific definitions
- Protects a surgeon's or dentist's hands, a nurse anesthetist's fine motor skills, an optometrist's vision and more
Student & trainee programs
Lock in coverage during training, often with discounts and simplified underwriting.
- Coverage that follows you after training
- Future purchase options to grow benefits
Future increase options
Add coverage as your income rises without new medical underwriting.
- Plan for the income jump after training
- Keep pace with partnership income
Residual & partial benefits
Partial benefits if an illness or injury reduces your hours or income without stopping you entirely.
Cost-of-living adjustment
Benefits that rise with inflation during a long-term claim.
Group & practice coverage
Review of employer and group plans, and supplemental policies to fill the gaps they leave.
Individual policy vs. employer group plan
| Feature | Individual own-occ policy | Typical employer group plan |
|---|---|---|
| Definition of disability | Your specific occupation or specialty | Often changes to "any occupation" after a set period |
| Portability | Yours if you change jobs | Usually ends when you leave the employer |
| Benefit taxation | Generally tax-free when you pay premiums with after-tax dollars | Often taxable if the employer pays |
| Benefit cap | Sized to your income | Frequently capped well below a healthcare provider’s income |
Before you sign: we review policy contracts line by line, including mental health limitations, exclusions and riders, so you know exactly what you are buying.
Life Insurance
Coverage sized to what your family would actually need: replacing your future earnings, paying off student loans and a mortgage, and funding your children's education.
Term life
Affordable, high-limit coverage for the years your family depends on your income.
- 10, 20 and 30-year terms
- Conversion options
Permanent life
Whole and universal life for estate planning, business succession and lifelong needs.

Buy-sell funding
Coverage that funds partnership agreements so a practice can continue if a partner dies.
How we size your coverage
Income replacement
Years of earnings your family would lose.
Debts
Student loans, mortgage and practice obligations.
Future goals
College, care for parents and other commitments.
Existing assets
Savings and group coverage that reduce the need.
Investments & Wealth Management
Healthcare providers often start saving later than their peers. We start with comprehensive financial planning on the eMoney platform, build disciplined, low-cost portfolios, add alternative investments where they fit, and bring your debt, taxes, insurance and estate together in one plan.
Investment guidance
Making the most of every account available to you.
Employer retirement plans
Contribution and fund strategy for 401(k) plans at private employers and, at nonprofit hospitals, universities and health systems, 403(b) and 457(b) plans. Includes Roth 401(k) and Roth 403(b) options.
Roth strategies
Backdoor Roth IRA, mega backdoor Roth where your 401(k) or 403(b) allows it, and Roth conversions, so high earners still build tax-free savings.
Practice retirement plans
Solo 401(k), SEP-IRA, profit-sharing and cash balance plans for practice owners.
Asset allocation
Diversified portfolios matched to your timeline and risk tolerance.
Tax-efficient investing
Asset location and tax-loss harvesting across taxable and retirement accounts.
Second-opinion reviews
An objective look at an existing portfolio, including fees you may not know you are paying.
Starting later is normal in healthcare. A provider who finishes training in their 30s has fewer years to compound. Our plans account for that with a higher savings rate and the right order of accounts.
Wealth management
One coordinated plan, reviewed with you as your career changes.
A complete financial plan, powered by eMoney
Every plan starts with comprehensive financial planning. We use eMoney, a widely used professional financial planning platform, to bring your whole financial picture into one place and keep it current.
- One live view. Accounts, insurance policies, student loans, mortgages and practice assets in a single dashboard.
- What-if planning. Model buying a practice, paying off loans early, a new home or retiring sooner, and see the effect before you decide.
- Private client portal. Secure access to your plan and a document vault from any device.
- Updated every year. Your plan is reviewed and adjusted at each annual meeting as your career changes.
Student loan strategy
Public Service Loan Forgiveness, income-driven repayment and refinancing decisions modeled against your career plans.
Contract review
Help evaluating compensation models, productivity targets, signing bonuses and non-compete terms.
Tax planning
Coordination with your CPA on entity structure, deductions and timing.
Asset protection
Strategies that shield personal wealth from professional liability.
Estate & legacy
Wills, trusts and charitable giving, coordinated with your estate attorney.
Practice transition
Buying into a practice, selling a practice and planning retirement income.
Alternative investments
Opportunities beyond stocks and bonds that can add diversification and income for qualified investors.
Private real estate funds
Professionally managed funds that own multifamily, industrial, medical office and other income-producing property.
Private credit
Funds that lend directly to businesses and real estate projects, with the potential for steady income.
Private equity & venture
Access to private companies, including healthcare and life sciences ventures where healthcare providers bring real insight.
1031 exchanges & DSTs
Defer capital gains when selling investment property by reinvesting through a 1031 exchange or Delaware Statutory Trust.
Opportunity Zone funds
Investments in designated communities that may offer tax benefits on reinvested capital gains.
Interval funds & structured notes
Options that offer exposure to alternative strategies with defined terms and periodic liquidity.
Alternatives are not for everyone. Many are available only to accredited investors, can be illiquid for years and carry higher risk and fees. We help you decide whether they belong in your plan and how much of your portfolio to allocate.
How we work with you
Discovery
We learn your goals, specialty, income and obligations.
Analysis
We review every policy, account and loan you hold.
Your plan
A written plan with clear priorities and costs.
Ongoing review
Regular check-ins as your career and family change.
Real Estate & Mortgages
From your first home to a portfolio of investment properties and the building your practice operates in, we arrange financing built around how healthcare providers earn.
Professional mortgages for your home
Large student loans and a new contract can make a standard mortgage hard to get. Professional loan programs for physicians, dentists and other healthcare providers are built for exactly that situation.
Low or no down payment
Programs for eligible healthcare providers that require little or no down payment, often without private mortgage insurance.
Student loan treatment
Lenders that consider deferred or income-driven student loan payments more favorably.
Close on your contract
Qualify with a signed employment contract, sometimes before your start date.
Refinancing
Lower your rate or restructure your loan as your income grows.
Relocation support
Help when you move for training, a new position or a new practice.
Jumbo & conventional
Competitive options for established providers buying higher-value homes.
| Feature | Professional loan | Conventional loan |
|---|---|---|
| Down payment | Often 0–10% for eligible providers | Typically 3–20% |
| Mortgage insurance | Often not required | Required below 20% down |
| Student loans | Favorable treatment of deferred loans | Standard debt-to-income rules |
| Income verification | Signed contract may be accepted | Usually requires pay history |
Real estate investment financing
Real estate can diversify your wealth and provide income outside clinical work.
Residential rentals
Financing for single-family rentals, two-to-four unit properties and vacation homes.
Multifamily & commercial
Debt placement for apartment buildings, retail, industrial and mixed-use properties.
Medical office buildings
Own the building your practice occupies and build equity instead of paying rent.
Practice acquisition
Financing to buy into or acquire a practice, including the real estate.
Syndications & partnerships
Guidance on passive investments alongside other healthcare providers.
Bridge & refinance
Short-term bridge loans and long-term refinancing to stabilize and hold assets.
From first conversation to closing
Review the deal
We assess the property, your goals and the numbers.
Structure
We match the right loan type and terms to the property.
Place the loan
We take your loan to banks, credit unions and private lenders.
Close
We manage the process through closing so you can keep practicing.
Terms vary by lender and borrower. Program availability, rates and eligibility depend on your profession, credit, location and loan amount. We compare options across lenders so you can choose with confidence.
Dentists & Other Practitioners
Dentists and other licensed healthcare professionals face many of the same challenges as physicians: years of training, significant student debt and a career that depends on specialized skills. We bring the same coordinated approach to your practice and your family.

Purchase or refinance the building your practice operates in, from a single dental suite to a multi-tenant medical building.

Run by physicians, for healthcare providers
Practice Wealth Strategies was founded by a physician and two veteran financial professionals who saw healthcare providers receive advice that ignored how healthcare careers actually work: long training, heavy debt, a late and steep rise in income, and real liability risk.
Dr. Akiva Marcus
Dr. Akiva Marcus, MD, PhD, is Founder and CEO of Practice Wealth Strategies. He is Chief of Medicine at HCA Florida JFK North Hospital and has practiced gastroenterology and hepatology in the Palm Beaches since 2015, after completing his gastroenterology fellowship at Montefiore Health System in New York. Dr. Marcus is also a co-founder, Chief Medical Officer and director of AmnioChor, Inc., a biotechnology company working on perinatal stem cell therapies, and serves on the board of Friends of Jerusalem College of Technology. Having built both a clinical career and a business, he founded Practice Wealth Strategies so that healthcare providers get financial guidance from people who understand how medical careers really work.
